Key Responsibilities:
- Patient Care: Review and monitor patient medication regimens to ensure appropriate therapeutic outcomes. Work with the healthcare team to develop and adjust medication therapies.
- Consultation: Provide medication counseling to patients and healthcare providers, ensuring safe and effective use of medications.
- Medication Management: Oversee drug utilization reviews, formulary management, and medication safety practices across the institution.
- Education: Educate patients and healthcare teams on the proper use of medications, potential side effects, and new treatment protocols.
- Clinical Support: Assist in the development and implementation of clinical guidelines, protocols, and policies to improve patient care and safety.
Qualifications:
- Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Licensure: Active Illinois Pharmacist license in good standing (or eligibility for licensure).
- Experience: Clinical pharmacy experience preferred, particularly in hospital or outpatient settings.
